DEW26M, now that is scarey

Any piece of heavy equipment I’ve ever been on, rather it be at a shipyard, or a commercial construction job was equipped with a form of parking brake, and countless other safety features. Rather or not they were properly adjusted or used is another issue. Many times the brakes did not properly engage, causing an extremely hazardous situation.I recall one time after stepping off of a LULL Forklift pictured here: http://www.nleq.com/equipimages/74000-1.jpg, that the brake did not engage, and just as I turned around the forklift came rolling backwards towards me. Had I not looked up when I did, it would have smashed me. I took it to the mechanic shop, and was told the parts were “ON ORDER” but they could not afford to place the lift “out of service”. Just be careful, I was told. I wonder if maintenance played a part in this case, or if it was operator error?

Carl hit it right on the head. This is such a sad situation. Safety awareness and proper training could save countless lives from being lost on the job. I have investigated numerous incidents involving heavy equipment with improper to no training provided to the operator and found this to be a huge problem. I’m not assuming this is the case in this incident, but whether it’s operator error, equipment malfunction or the error of another party, not only the family suffers but also the other workers who had to witness their buddy’s death.

We always lose a few on major construction jobs and it’s never easy to take and the majority of the times it could have been prevented with the appropriate safety steps observed. No doubt an increase in safety awareness will now be touted but it wont bring James Hall back or ease the suffering of his family. The death of any one is a little piece of death for me. My heart felt condolences to his family and friends.
